All our care is delivered in line with the Care Act and lets service users lead their support. This complies with the Mental Capacity Act legislation which focuses on capacity in decision making.
We treat each individual with equality and respect as we continuously monitor progress and make adjustments to the care plan when necessary. This involves regular team meetings and consultation with stakeholders including the service user and their family and friends to ensure care remains relevant and suitable for needs.
Information is shared securely and in compliance with our data protection standards. We use the decision-making profile to ensure we understand how we can support the individual to lead their care and support effectively.
Personalised risk management plans outline strategies for mitigating identified risks while maximising opportunities for growth and focusing on strengths based approaches.
