Using the model of reablement, we focus on the strengths of each individual to maximise their independence and wellbeing and reduce reliance on services. Our care teams influence positive change through joint objective planning and tailored support.

We work to remove barriers and create an environment where everyone feels valued, supported and part of their community.
Our support enables individuals who have experienced mental health issues such as anxiety, depression, trauma or behavioural disorders, to re-learn skills to engage in activities that are important to them. This is especially important for those who have recently left hospital or have been away from their home for some time.
We implement this by delivering support that is:

Focusing on individual strengths and capabilities and allowing these to lead support.

Preparing for potential risks and supporting risk management through effective care planning.
To apply this successfully, our care teams are well structured, trained appropriately and have the foundation of reablement as a method of support. In addition, we gain further support from specialists in reablement including occupational or physiotherapists as well as the family/carer of the service user.
We review progress in this model more often than standard care to ensure the outcome focused approach remains aligned with needs.
We want each person to thrive in our care and feel supported and able to talk to their care team should they feel they are struggling with their mental health. We offer additional support such as therapies to explore emotions, develop coping skills and work through personal challenges. In times of crisis, we offer immediate crisis intervention to ensure safety.
